چکیده
T he main complication of anticoagulation therapy is bleeding. We have conducted a literature review to determine: (a) the rate of bleeding during long-term anticoagulant therapy in various clinical disorders, and (b) the clinical and laboratory risk factors that predispose patients to bleeding. For the purposes of this review, we defined long-term therapy as lasting greater than 4 weeks. Hemorrhagic complications were also described for 2 other indications, myocardial infarction and hip surgery, where oral anticoagulant therapy was used for a shorter duration (approximately 4 weeks). The analysis was performed in patients with: (1) ischemic cerebrovascular disease (completed stroke, stroke-in-evolution, and transient ischemic attacks); (2) prosthetic cardiac valves; (3) chronic atrial fibrillation with and without associated rheumatic valvular disease; (4) ischemic heart disease (angina, myocardial infarction); and (5) in the prevention and treatment of venous thromboembolic disease. We reviewed each report seeking the following information:

Anticoagulant and Antiplatelet Use in Cataract Surgery and Combined with Posterior Vitrectomy
Patients receiving long-term anticoagulant and antiplatelet medications pose a clinical challenge when therapy needs intraocular surgery, including cataract surgery and vitrectomy [1-3]. Maintaining antiplatelet and anticoagulation places them at risk for serious bleeding complications, whereas discontinuing these medications puts them at risk of thromboembolic complications [4-6]. Currently, t...

Our experience with thromboembolism in 283 patients surviving at least 1 week following insertion of Starr-Edwards valves is reported here. Of these patients, 155 underwent aortic valve replacement, 21 had aortic valve replacement with mitral commissurotomy, 80 had mitral replacement, and 27 had both aortic and mitral valve replacement. Complete follow-up data were obtained on all patients, fro...
متن کاملComments, Opinions, and Reviews Hemorrhagic Complications of Long-Term Anticoagulant Therapy for Ischemic Cerebral Vascular Disease
The main complication of anticoagulant therapy is bleeding. Although the use of long-term oral anticoagulants in patients with transient cerebral ischemia and/or minor stroke is controversial, anticoagulants are still used in some instances. We have carried out a literature review of the risk of hemorrhage during long-term oral anticoagulant therapy in patients with cerebrovascular disease to d...
متن کاملProblems Associated with Long - Term Anticoagulant Therapy
LONG-TERM anticoagulant therapy is being used frequently in the management of certain vascular and thromboembolic diseases. Results predominantly favorable in respect to prolonged survival and decreased morbidity have been reported from use of coumarin anticoagulants in these diseases.1-4 However, there has not been adequate evaluation of the ease or difficulty with which a satisfactory and pra...
